AUROFORM strictly adheres to the principle of data minimization. We do not collect medical records, protected health information (PHI), or patient identifiers. All files transferred for design must be anonymized by the clinic or laboratory beforehand.

2. What data we collect via the form

The checklist request form collects only the minimal necessary B2B business contact details:

• Contact name;
• Clinic or laboratory name;
• Professional role;
• Work contact (email or messenger handle).

3. Purpose of processing

Contact details are used exclusively for:

• Handling your request for the PDF checklist;
• Responding to a direct B2B inquiry regarding CAD design services.

We do not run spam mailing lists. Contacts are transmitted exclusively through the configured delivery infrastructure (Cloudflare Pages Functions, Telegram Bot API, or webhooks) to handle and respond to your request. We do not sell or share contacts with advertising networks.

4. Website analytics

No website analytics script runs on this site. The checklist form uses the contact details described above to handle your request.

5. Consent withdrawal and data erasure

You can withdraw your consent and request complete deletion of your contact data at any time by writing to auroform.dental@gmail.com.

6. Scope of responsibility (design-only)

AUROFORM provides digital CAD design services exclusively for exocad DentalCAD. We do not manufacture or print surgical guides or prosthetics, and we do not issue clinical treatment plans. The final clinical decision and mandatory pre-manufacturing verification remain the sole responsibility of the clinician and laboratory.
